DHS, OLTL, UPMC & Beyond

This page exposes the broader systemic failures across various government and healthcare agencies. It details a relentless cycle of mismanagement, misinformation, and neglect that has actively prevented me from receiving the care and services I am entitled to as a disabled citizen. Each point below represents a wall I have had to face, often simultaneously.  Waiver Service Application Failure (2023): A detailed chronology of my first waiver application, marked by lost paperwork, misinformation from the Area Agency on Aging, the 'erasure' of my physician's form, and a verbal denial without any formal letter, leaving me without critical PCA support.  UPMC/Lori/APS Grievance (June 2025): A UPMC coordinator, Lori, made a false police report that I was suicidal, leading to an unwarranted police presence at my home. This mischaracterized my needs, violated my privacy, and demonstrated gross professional misconduct. UPMC later refused to provide discovery for my grievance hearing.  BEO & OLTSS Grievance Confusion (June-July 2025): In desperation, I submitted a grievance intended for OLTSS through the My Compass app, which was unknowingly routed to the BEO. This led to a confusing questionnaire and a lack of direct response to my urgent issues with the CAO and my Part B benefits. The BEO recently sent notice stating their intrests in investigating OLTL and UPMC.  As I am trying to find legal representation to assist in all these simultaneously wrong doings from the goverment entities, I now have to complete another packet of questioning directed to those said departments. The ongoing ISP  (February 2024 - Present): Internet Service Provider (Astound) has consistently failed to provide reliable service in regards to RTT, impacting my job and well-being, while lying about service quality.  This has been the biggest factor which has subsecuently caused my breaking point.  Not being able to communicate or use of my smart devices has left me in solitude.
